Documents you will need

• Aadhaar card
• PAN card
• Passport-size photographs
• Proof of the reason for the change, where applicable
• Marriage certificate, if the change follows a marriage

Scans or clear photographs are fine. The lawyer handling your matter will tell you if anything else is needed once they have seen your papers.
